On October 29th, 2020 Governor Sununu announced that restaurants will be required to collect contact information for patrons dining at their establishments beginning on October 31st. The most recent guidance can be found by clicking here. We have created a flyer with this information and a data collection sheet you may use for this purpose, ready for download below. This information must be retained for at least 21 days and only for the purposes of contact tracing in the event a positive COVID-19 case is identified at your establishment.
